Your role will involve working independently as an autonomous practitioner and with the wider multidisciplinary team, providing specialist assessment and intervention to individuals presenting with a range of physical, cognitive, social, or perceptual deficits that impact functional ability and safety within their home environment. You will ensure a proactive approach to rehabilitation to maintain and improve function and facilitate timely discharges, always thinking 'Home First'.
