Behavioral Health Case Manager

We’re hiring a Behavioral Health Case Manager to help patients access, remain engaged in, and transition between the services they need. This role will support patients with complex behavioral health needs, including those moving between levels of care, while maintaining the flexibility to support outpatient psychiatry and therapy. You will work closely with patients, clinicians, internal program teams, insurers, and external partners to remove barriers, coordinate services, and close the loop on referrals and transitions. You will not provide psychotherapy, diagnose patients, or make independent clinical or level-of-care decisions. You will be expected to recognize changes in risk or acuity and escalate promptly to the appropriate clinician.
