Position Summary Open to search in Houston, TX Nurturing Home Healthcare is seeking a dedicated and compassionate Physical Therapist (PT) – Home Health (PRN) to provide skilled physical therapy services to patients in their homes. The Physical Therapist will evaluate patients, develop individualized treatment plans, implement therapeutic interventions, and work collaboratively with physicians, caregivers, and interdisciplinary team members to improve patient mobility, function, safety, and quality of life. The ideal candidate is passionate about patient care, possesses strong clinical judgment, and enjoys working independently while delivering exceptional therapy services in a home health setting.
