Job description
Position Objective: Provides psychosocial assessment, individual, family and group therapy to case load of patients. assessments, plans for and implements identified services for patients. Works with the patient and family/significant others on a safe discharge plan. Collaborates with the provider and interdisciplinary team on the care plan. Essential Job Duties: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions In accordance with established department and professional guidelines, provides comprehensive psychosocial assessments for patients on case load. In therapy role, creates a therapeutic framework for effective problem solving with patient and family. Responds to patient/family needs related mental health issues.
